Lost passport report and replacement route

Report a missing valid U.S. passport to the U.S. Department of State through its online Form Filler. Reporting cancels the passport and does not order a replacement. Keep those two steps separate when deciding what to do before your next trip.

How do I report a lost or stolen U.S. passport?

Use the lost-or-stolen report in the State Department Form Filler. Supply the requested information about the physical passport and loss. Keep the confirmation email. If using the mailed DS-64 option, print and sign the form, include the required photo-ID copies, and follow the address printed on the form.

Online reporting cancels the passport within one business day. A mailed report can take weeks to arrive and be processed. Do not treat placing an envelope in the mail as confirmation that the missing document has already been canceled.

Can I use my passport if I find it after reporting it lost?

A passport canceled after a loss report cannot be used for international travel, even if it turns up later. Finding it does not restore its validity. Make replacement arrangements rather than taking the recovered document to the airport as your travel passport.

Do I report a lost expired passport?

The State Department says not to report an expired passport as lost or stolen. Check which physical document is missing and whether it is still valid before selecting a loss report.

An expired book and a separately issued card are distinct documents. Read the instructions for the document actually lost instead of reporting every passport product you have held.

Do I need DS-11 as well as DS-64?

DS-64 reports a loss. DS-11 applies for a new passport. To replace a lost passport, follow the in-person DS-11 application route. Report the circumstances of the loss on the application as instructed. If important details are missing, the agency can request DS-64 and pause processing.

A loss report is not a renewal application. Do not pay for a renewal service expecting it to replace a passport that you cannot submit or have reported missing.

What if my new passport was lost in the mail?

First check the application status and delivery record. If the passport has not arrived more than two weeks after mailing, contact the National Passport Information Center at 877-487-2778 about non-receipt. That process uses DS-86 and differs from reporting a passport you received and later lost.

FAQ

Does reporting a lost passport automatically replace it?

No. You must separately apply in person for a replacement.

Can I renew a passport already reported lost?

Use the in-person application route. A passport reported lost or stolen does not qualify for the usual renewal process.

Should I report only an exposed passport number?

The State Department distinguishes number exposure from loss of the physical document. Use the loss report when the valid physical passport is missing or stolen.
